A fatal Legionnaires’ disease outbreak in South Carolina was caused by two novel sources -waterjet cutters and floor scrubbers, according to a new report from the Centers for Disease Control and Prevention (CDC). Legionnaires’ disease, a severe form of pneumonia, is transmitted when people inhale water vapor contaminated with Legionella. Outbreaks are most often associated with fountains, hot tubs, cooling towers, and the plumbing systems of large buildings. Never before has an outbreak been linked to these devices.

Between August 2022 and November 2022, the outbreak sickened 34 people, hospitalizing 15 of them. Two people died. The illness cluster occurred at an unnamed manufacturing facility in Richland County, South Carolina. Of the total patients, 10 had confirmed Legionnaires’ disease, 20 had probable Legionnaires’ disease, and four had probable Pontiac fever, a milder form of Legionnaires’ disease.
Tests showed that the bacteria cultured from patients matched those found in samples taken from two waterjet cutters and one floor scrubber. Researchers concluded that “manufacturing facilities that use aerosol-generating devices should consider maintaining updated Legionella water management programs.”
Symptoms of Legionnaires’ Disease
Symptoms of Legionnaires’ disease usually develop within 14 days of exposure and are similar to the flu. They include:
- Fever
- Cough
- Shortness of breath
- Headache
- Muscle aches
Some people, including current and former smokers, people over 50, and people with weakened immune systems are at elevated risk of contracting Legionnaires’ disease. Most people with these infections require hospitalization. About 10 percent of cases are fatal.
